Prevention of serous carcinoma in the general population

Summary

In Chapter 9, the main results of this thesis are discussed. This chapter describes recommendations for daily practice, policy development, and future research.

From the previous chapters, we conclude that opportunistic salpingectomy (OS) is increasingly being performed during gynecological surgeries, but is not yet a standard procedure for non-gynecological abdominal surgeries. Providing counseling and performing OS during gynecological surgeries depends on the type of surgery, the surgical approach, the reason for the surgery, and the physician. The use of a patient decision aid increased women's knowledge of ovarian cancer and OS. Most patients chose OS after using the decision aid and experienced little stress.

Implementing OS during non-gynecological surgeries can prevent even more cases of ovarian cancer and potentially save costs in the long term. For a successful implementation of OS in non-gynecological surgeries, it is necessary to develop a plan that considers barriers and facilitators. Training for surgeons and urologists on counseling and performing the procedure can assist in this. It is also important to have clear educational materials and guidelines regarding which patients are eligible for OS. The additional time required for counseling and surgery, along with potential complications, should be investigated. Clarity regarding the costs and reimbursements of OS can facilitate and accelerate its implementation.
